The Home Health Physical Therapist Assistant will administer comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.Principal ResponsibilitiesFollow plan of care developed by Physical Therapist using a variety of treatment techniques.Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordinationConsult and practice with other health professionals, involved with patients care.Provide quality, personalized, evidence-based care, and proven interventions, specific to patient's needs.Encouraging and help motivate the patient during their recovery process.Completes required documentation to complete the patients visits and episodes, as required by federal, state and payor guidelines.Instructs patients and family/significant others in home exercise programs, to facilitate function.Education/ExperienceSuccessful completion of a Physical Therapy Assistant program approved by CAPTE, Commission on Accreditation in Physical Therapy EducationOne year of Outpatient, SNF, or Inpatient Rehab care experience is required.Associate degree in Physical Therapy AssistantMust have dependable transportation, possess a valid state driver's license and automobile insuranceLicenses/CertificatesCurrently licensed by the Illinois State Board of Physical Therapy.Current AHA CPR certification is required.Reliable means of transportation, valid IL driver's license and vehicle liability insuranceKnowledge, Skills and AbilitiesProfessional behavior and communication skillsStrong communication skills, flexibility, and professionalismSelf-direction with a strong sense of team commitmentStrong organizational and time managementWorking EnvironmentThis job operates in a variety of settings, including but not limited to professional office environment, patient's homes, and car.
