ProMedica Senior Care, formerly HCR ManorCare/Heartland, provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.

ProMedica Hospice is currently searching for a less than part-time Spiritual Care Coordinator to support hospice patients, families and staff. This position offers a flexible schedule. Weekday hours available with an after hours on call rotation.

Responsible to provide spiritual support to clients, families, and the hospice staff via phone contact, home visits and referrals to the client/family, local clergy. Assists with memorial services and activities relating to spiritual and religious issues of client, family and interdisciplinary team.

Educational Requirements

Bachelor's from an accredited school of theology or equivalent, or Master's in Counseling or Master's in Social Work with 2 units of Clinical Pastoral Education (CPE) required; Master's in theological studies or equivalent and completion of 2 units of CPE preferred. If affiliated or endorsed by a denomination or faith group must be in good standing, as evidenced by a certificate.

  • Ability to develop and maintain rapport with caregiver and patients.
  • Demonstrated experience in working with patients and caregivers coping with terminal illness and death.
  • Excellent communication skills, both verbal and written. Excellent interpersonal skills.
  • Demonstrated problem solving and decision-making abilities.
  • Knowledge of Hospice philosophy/palliative care and Medicare Hospice benefit rules & regulations.

    Experience
    • Experience in patient/caregiver counseling.
    • Proficient computer skills preferred

      Travel
      • 100%; necessary daily.

        • Exposure to disagreeable conditions including odors, infection, illness, and physical contact from disruptive patients.
          Job Specific Details:

          General Responsibilities
          • Provides spiritual support & counseling to patients and caregivers as related to issues of grief, loss & the dying process.
          • Contacts patient/caregiver, clergy or spiritual representative when appropriate. Works in collaboration with community clergy as desired by patient/caregiver.
          • Conducts funeral services when requested by the patient?s caregiver.
          • Assists with bereavement follow-up services as appropriate.
          • Participates in on-call Spiritual Care Coordinator services.
          • Provides spiritual support to the interdisciplinary team.
          • Submits paperwork in a timely manner to update and maintain patient medical records. Documentation is clear & complete.
          • Prepares reports for the Director of Professional Service and/or Patient Care Coordinator, as required.
          • Participates in regularly scheduled interdisciplinary team meetings to coordinate the spiritual care of the patient/caregiver, exchange information and problem solve, receive staff support and education, and evaluate patient/caregiver response to call provided.
          • Participates in agency quality assessment performance improvement (QAPI) programs.
          • Participates in programs providing orientation, training and inservice. Serves as a resource to interdisciplinary team.
          • Provides programs to the community on hospice and related issues of loss.
          • Accepts other assignments deemed appropriate.
          • Directs or performs spiritual assessment of patients & caregiver members as soon as possible upon admission.
          • Develops and implements individualized spiritual plan of care based on assessment & consultation with other team members. Updates plan as changes occur.
          • Assists patient, caregiver & staff in acquiring & utilizing community resources, as needed.
          • Adheres to established personnel policies.
          • Conducts religious services and/or rituals for patients/caregivers, as appropriate.
          • Offers patients/caregivers with non-traditional philosophies/beliefs opportunity to discuss and share their beliefs and values.
          • Communicates patient changes in condition to appropriate staff.
          • Willingly accepts direction from Director of Professional Services and Patient Care Coordinator.
          • Communicates patient/caregiver needs to members of interdisciplinary team.
          • Schedules visits with patient/caregiver prior to the visit and confirms appointment, as needed. Visits are scheduled according to patient/caregiver need.
          • Completes spiritual assessment as indicated and appropriate as soon as possible after admission.
          • Provides spiritual support in accordance with on-going wishes and needs of the patient/caregiver.
          • Develops an individualized spiritual plan of care which demonstrates an effort to work in close collaboration with local clergy whenever desired by the patient and provides spiritual support as needed and defined by the patient/caregiver.
          • Conducts religious services of prayer, worship and rituals for patients/caregivers, as appropriate.
          • Documents counseling provided in the patient?s clinical record.
          • Works with other professionals in resolving spiritual/pastoral/ethical issues.
          • Works in a team approach with members of the IDT/IDG and other qualified professionals, as determined by the hospice, to evaluate patient/caregiver response to care.
          • Attends IDT/IDG conferences and reviews plan of care.
          • Assists in developing and updating the patient/caregiver plan of care.
          • Assesses patient/caregiver response to and satisfaction with care.
          • Participates in development of approaches to meet staff support needs.
