The main focus of the QIDP--position is to coordinate the development and implementation of supported living services with referral sources, clients--and employees, while ensuring the delivery of quality services. This coordination occurs through the direct supervision of employees, support--for clients, and ongoing communication with family, community, and funding and licensure sources. The QIDP will be responsible for--case management for 12 or more individuals, creating care plans, conducting care assessments, assisting with individual daily care plans, and training staff. They will also be responsible for attending care appointments and--providing direct care services, as needed.--This position reports directly to the Supportive Services Director or Branch Director.Benefits* Health, dental and vision insurance* Holiday pay* Paid time off* Paid vacation* Mileage reimbursement* Paid travel time* Short-term and long-term disability options* Supportive management* Team-oriented environmentResponsibilities* Person Centered Planning--and Individual Support Planning:* Actively participate--in the person centered planning process, including development and implementation of Person Centered Plans (PCPs) and the Individual Support Plan (ISPs)* Design and implement support and teaching strategies targeted at assisting clients with reaching their outcomes * Provide supervision and training to Direct Care Staff regarding the implementation of the PCP, ISP and teaching strategies* Evaluate and revise support and teaching strategies, as necessary, to ensure progress toward achievement of outcomes and action steps* Regularly monitor the implementation of the PCP and ISP* Revise PCP and ISP outcomes, as necessary, based on data collection, feedback from staff, and direct observation of the client* Complete monthly outcome report and provide to the Case Manager* Facilitate opportunities for natural support* Document specific QIDP activities provided to the client* Client Records:* Maintain client--files including, but not limited to:* Client profile data with emergency contact numbers* Copies of Medicare/Medicaid cards for all supported living sites, current Release of Information forms, consent forms, and current signed Consumer Rights statement* Copies of guardianship paperwork (if applicable)* Financial records in the Help at Home Inc. financial system, including documentation of all account spending, personal spending, and household spending* Daily progress notes, monthly reviews, and other client-related documentation* Medical information including documentation of medical appointments, lab results, immunizations, medication orders, Medication Administration Records, and controlled drug count sheets* Monthly Health Summary* Quality Assurance:* Maintain confidentiality of all clients' Protected Health Information, as mandated by HIPAA* Review all forms, notify Case Manager of any incidents and provide oversight for--follow-up plans with staff* Maintain communication with family members, guardians, staff and Case Managers on a regular basis, particularly regarding policies, issues, and concerns* Prepare and submit reports that are required by contract, funding sources, or regulations* Supervision/Training:* Coordinate in-service trainings * Monitor and record all required trainings--for staff, while scheduling updates, as needed* Provide direct support by--providing services needed to protect the health and safety of the client in a staffing crisis* Communicate with the Interdisciplinary Team about concerns and findings* Be available to respond to staff in case of an emergency* Other Duties:* Assist with coordinating and managing staffing schedules as needed* Coordinate--client benefits including Medicaid, Medicare, SSI, SSA, and Food Stamps* Secure community resources for clients * Ensure--compliance with licensure guidelines* Assist the Director with--teaching training classes, as requested* Assist the Director with on-site investigations, as requested, for worker injury and client complaints* Assist with job interviews and reference checks, as directed by Director* Maintain and promote professional manner for self--and support staff within all communications* Assist the Director with other duties, as assigned and with approvalMinimum Qualifications* Bachelor's degree--in social work, psychology, human services or a related field * At least one year of experience working with adults with developmental disabilities* QIDP Certification--(can be obtained through HAH) * Supervisory experience--preferred* Experience coordinating and managing staffing schedules* A personal vehicle that is insured and meets state safety/licensure requirements* Valid driver's license* Availability to work day, evening and weekend shifts* Availability to be On-Call* 24/7 availability for caseload-related crisesIn order to be employed as a Qualified Intellectual Disabilities Professional, candidates must comply with state background screening requirements.
