Job Description:
Overview
The AMS pharmacist provides direct patient care including
management, monitoring, and instruction related to anticoagulation
management. The pharmacist will provide high quality patient care
through a collaborative practice agreement for anticoagulation
management of patients referred by any Carle Provider to AMS. They
will be responsible for implementation, monitoring, and overall
management of anticoagulation therapy regimens. They will provide
extensive patient education on warfarin therapy. They will work to
ensure appropriate and optimized anticoagulant therapy and
bridging, and provide education to patients as well as other
healthcare professionals. They will provide direct supervision and
instruction to AMS pharmacy technicians.

Certifications: Licensed Pharmacist - Illinois Department of
Financial and Professional Regulation (IDFPR), Education:

Responsibilities
Work in collaboration with other healthcare professionals to
provide safe, effective anticoagulation management. Implement,
monitor, and adjust anticoagulation and peri-procedural bridging
therapies for warfarin and direct oral anticoagulants. Directly
supervise AMS staff, (i.e. technicians) in AMS support functions
Assist in the development of clinical guidelines and order sets as
related to appropriate drug therapy. Provide direct patient care by
intervening as appropriate to improve prescribing, administration,
and monitoring practices. Represent the pharmacy on committees or
work groups as assigned. Screen medications for potential
interactions with other medications, diet, disease state, or stated
allergies. Make dosing and therapy recommendations based on
patient-specific factors, including age, weight, renal function,
and clinical status. Assist in the training and orientation of new
employees as assigned. Provide Medication Therapy Management
services as defined in the Illinois Pharmacy Practice Act Maintains
current knowledge of and ensures compliance with government and
commercial regulations. Other duties as assigned. Review all AMS
INRs and assign calls per protocol (alert, today, 24 hours, 24-48
hours). Call and triage patients with abnormal INR lab results,
assess clinical status, and determine appropriate next steps.
Manage patient's warfarin per protocol or collaborative practice
agreement. Provide patient education related to anticoagulation
therapy, INR results, dosing instructions, and safety
considerations. Provide pre and post procedure guidance; assist in
transitions of care, including coordination of hospital discharges
with the inpatient pharmacy team. Manage excellent patient
communications including telephone messages/patient encounters for
questions, triage, and education.

Compensation and Benefits
The compensation range for this position is $52.06per hour -
$89.54per hour. This represents a good faith minimum and maximum
range for the role at the time of posting by Carle Health. The
actual compensation offered a candidate will be dependent on a
variety of factors including, but not limited to, the candidate's
experience, qualifications, location, training, licenses, shifts
