Job Description:
Overview Responsible for coordinating day-to-day clinical
services of the SOR Hospital Screening and Warm Handoff grant.
Qualifications Certifications: - Driver's License - Illinois
Secretary of State (ILSOS); - Basic Life Support (BLS) within 30
days - American Heart Association (AHA), - Must have control over
personal use of all mood altering chemicals and behaviors. Upon
Hire - If recovering from an addiction, must currently have a
minimum of 2 years of uninterrupted recovery and must actively
treat this addiction by appropriate recovery practices including.
Upon Hire - Use of usual and customary equipment used to perform
essential functions of the position. Upon Hire Education:
Bachelor's Degree: Counseling; Social Work; Nursing; Related Field.
Work Experience: Behavioral health. Other Requirements Advanced
reading, Writing and Oral skills Ability to respond appropriately
to customer/co-worker. Interaction with a wide variety of people.
Maintain confidential information. Ability to communicate only the
facts to recipients or to decline to reveal information. Ability to
project a professional, friendly, helpful demeanor. Basic computer
knowledge: Uses word processing, spreadsheet, e-mail application,
and web browser. Comfortable within Windows OS and learning new
applications. Ability to utilize an electronic medical record.
Responsibilities MONITORS AND COORDINATES SERVICES - Maintains
staffing patterns and day to day operations of SAMHSA-SOR grant. -
Provides monthly and/or as needed supervision and documents
response to supervision. - Develops and maintains work schedule for
clinical staff as needed. - Monitor admission and discharge
process. - Responsible for selection, retention and training of
personnel. - Supervises implementation of UnityPoint Health
Standards of Excellence by staff. - Abides by guidelines outlined
in the SOR Hospital Screening and Warm Handoff grant - Supervises
the initiation of standardized protocols for clinical care where
indicated. - Ensures that discharge planning is individualized in
accordance with the patient's/client's needs and requirements set
forth in the SOR Hospital Screening and Warm Handoff grant. -
Oversees the activities of the Certified Recovery Specialist, Case
Managers, and Data Analyst. - May be responsible for transporting
patients to appointments MAINTAINS EFFECTIVE COMMUNCIATION AND
TEAMWORK STYLE WITH COWORKERS, OTHER DEPARTMENTS, OTHER HEALTH CARE
PROVIDERS AND THE COMMUNITY. - Will assist in the development and
implementation of educational programs as needed and indicated in
the grant. - Actively participate in communication, attendance in
required meetings, and learning collaborative sessions with
IDHS/SUPR. - Actively maintaining professional relationship with
open communication with providers, units, clinics. - Actively
maintains open communication & demonstrates a cooperative, teamwork
style with patients, families, other staff members, other
healthcare workers, medical staff and community. - Demonstrates
organizational abilities through correct prioritization of duties,
efficient and appropriate usage of time, and effective utilization
of support personnel. Required Documentation for EMR and SAMHSA-SOR
Grant - Ability to understand data and provide analysis of the
data. - Collection and reporting of data to inform process
evaluation of the development, implementation, and functioning of
the project. - Maintains computer proficiency and audit reporting
of SAMHSA-SOR required data - Ensures accurate documentation -
Collaborates with Certified Recovery Specialist and Case Managers
to ensure documentation is present to supports warm hand off. -
Assist with documentation in EMR to support quality initiatives -
Assist with audits for medical necessity documents as requested -
Oversees and ensures that data is entered timely and correct in the

The compensation range for this position is $24.86per hour -
$41.52per hour. This represents a good faith minimum and maximum
range for the role at the time of posting by Carle Health. The
actual compensation offered a candidate will be dependent on a
variety of factors including, but not limited to, the candidate’s
experience, qualifications, location, training, licenses, shifts
