Assistant (PTA) – Travel Contract Unit: DMH-Forsyth Medical Rehab
Specialty: Outpatient PTA Duration: 13 Weeks COVID Vaccine: Not
Required Flu Vaccine: Required (Medical/Religious Exemptions Only)
Shift Details - Schedule: 5x8 Days (8:00 AM – 4:30 PM) Open to
4x10s as well - Weekend Requirement: None - On-Call: No - Block
Schedule: Monday–Friday (Blocked) - Guaranteed Hours: Client may
call off up to 36 hours per 13-week assignment Requirements -
Experience: Minimum 1 year required (First-timers not accepted) -
License: Active Illinois (IL) PTA License required at submission -
Certifications: BLS (AHA or Red Cross only – No RQI) - Floating:
Required within hospital or nearby affiliate facilities - RTO:
Ideally no more than 5 days (Non-holiday) - Major Holidays: Must
