Job Description:

delivery system. As a Pediatric RN-Nurse Navigator, you will guide
patients and their families through complex pediatric healthcare
processes, providing education, support, and coordinated care. This
role focuses on improving patient outcomes, facilitating effective
communication among care teams, and championing patient advocacy
and education. Act as a liaison between patients, families, and
various healthcare professionals. Develop and implement patient
care plans in collaboration with other health care providers.
Educate patients and families on diagnoses, treatment options, and
resources. Monitor patient progress and adjust care plans as
needed. Facilitate multidisciplinary team meetings to ensure
coordinated and comprehensive care. Document patient care and
interactions in medical records accurately. Strong clinical skills
in a pediatric healthcare setting. Excellent communication and
interpersonal skills. Ability to multitask and manage time
effectively. Strong advocacy and ethical decision-making skills.
Proficiency in electronic health record (EHR) systems. Medical
Insurance Dental Insurance Vision Insurance Life Insurance 401k
Current RN license in the state of practice. BLS/CPR certification.
Preferred: Certified Pediatric Nurse (CPN) or eligible.
