From pre-op education to discharge planning and follow-up, you’ll be the connector that makes the patient journey seamless. What You’ll Do: Support and guide care teams to deliver outstanding outcomes; Coordinate patient care plans and discharge planning. Promote best practices, safety, and efficiency across inpatient and outpatient settings. Serve as a resource and set the standard for professional excellence.
