Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty. License: Must have license in hand. Certifications: CPR, BLS Must-Have: Proficiency with HomeCare HomeBase and OASIS within the last 6 months, or completion of a retraining course during compliance. Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures. Responsible for the competent application of the nursing process. Accountable to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits and periodic office meetings are required. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day ranges from 5 to 8. Contractor will be assigned between 5 to 10 patients per day, equating to roughly 6 to 8 points. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Weekend requirements vary by location and profession and should be discussed during the interview. Potential on-call requirements may include 1 weeknight and 1 weekend per month. Will involve traveling up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Local applicants accepted at the same rate. No pending licenses are accepted; applicants must have their license in hand. Proficiency with HomeCare HomeBase and effective documentation is required. Expected weekly productivity points are 30. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation impacting billing and follow-up patient care may result in cancellations. There are no guaranteed hours with this contract. Clinician must accept all scheduled patients regardless of distance from previous appointments. If patients are insufficient for financial needs, there may be opportunities to float to another nearby clinic.
