Job Description:

The Home Health Registered Nurse is responsible for traveling to a patients home to provide skilled nursing service in accordance with the orders of the patients health care professional, under a plan of treatment established by the healthcare team.Principal ResponsibilitiesBe responsible for the observation, assessment, nursing diagnosis, council, care and health teaching for patients, and health maintenance and illness prevention.Provide progress notes to the patients provider when the patients conditions change or there are deviations from the plan of care, or at least every 60 days.Direct the activities of the licensed practical nurse and supervise the aide in the home.Administer medications and treatment as prescribed by the patients provider.Act as the coordinator of the healthcare team to maintain the proper linkages within a continuum of care.Perform acts in accordance with the IL Nurse Practice Act.Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.Attends all training and educational requirements for role.Education/Experience1+ year experience in a clinical care setting as a nurseAssociates Degree or Bachelors Degree in Nursing is preferredPrevious Home Health or Hospice Experience is preferredLicenses/CertificatesCurrent, unrestricted state RN licenseeCPR certification with American Heart AssociationMust have a dependable vehicle, valid drivers license, and current auto insurance in accordance with state laws.Knowledge, Skills and AbilitiesAble and willing to travel within branch/office coverage area.Working EnvironmentThis job operates in a professional office environment.
