Job Description:
Seeking a Nurse Practitioner (PRN) for home risk assessments,
welcoming new grads.
Conducts comprehensive adult/geriatric assessments in home, SNF,
and community settings, reviewing health, history, social factors,
and risks.
Collaborates with physicians, case managers, and caregivers to
develop care plans and provide follow-up.
Visits can last up to an hour, aiming to improve care quality and
close health gaps.
Offers flexible scheduling, including days, evenings, nights, and
weekends, with potential earnings up to $11,500/month based on
assessments completed.
Requirements include a Master--------s degree or equivalent,
current RN and NP licensure, board certification, and at least 1
year of NP experience.
Travel is required to meet patients at their locations. Benefits
include mileage reimbursement, 401(k), and voluntary benefits.
