Non_ExemptExemptDescriptionThis position will serve as a functional
member of the Quality Improvement Team in implementation of program
goals and objective. This role involves a systematic approach to
monitoring, assessing and improving health care quality at SIU
Springfield Center for Family Medicine. Primary functions will
include review of charts/processes, gathering/collecting data,
working reports, and assisting with the assessment and monitoring
of quality metrics. This position will work closely with the
Director of Quality and CMO and collaborate with physicians,
residents and other members of the health care team and patients to
promote the delivery of quality, efficient and cost-effective
healthcare services in a patient-centered environment.#IND1Examples
of DutiesAdministration of Quality Improvement - 90%A. Works with
healthcare teams across the FQHC to continuously evaluate
processes, identify problems, and propose improvement strategies to
enhance the delivery of care model. B. Collaborates with
clinicians, health system administrators/staff to design workflows
which promote best practices.C. Educates clinical staff regarding
national quality measure standards and algorithms to improve
performance, drawing from evidence-based practice and the
literature.D. Responsible for keeping abreast of quality measures
issued by NCQA Patient Centered Medical Home or HRSA for Uniform
Data System submission.E. Assists the Director of Quality in
developing performance feedback mechanisms that address process and
behavior as part of improvement efforts.F. Prepares periodic
progress reports to leadership and identifies barriers, develops
action plans and issues requiring escalation for resolution.G.
Assists Director of Quality with coaching teams on how to conduct
rapid tests of change to evaluate the effectiveness of implemented
changes in practice or protocol.H. Participates in the design of
team audits to measure compliance of clinicalpractice/operations,
utilizing appropriate sampling methodologies.I. Assists Director of
Quality with educating team members in the application of the Model
for Improvement, coordinating QI team efforts and assisting team
leads with optimizing team dynamics.J. Works with support staff and
informatics nurse to assist in identifying and implementing reports
or processes that improve follow-up of care, such as laboratory and
diagnostic testing, utilization patterns and other metrics to
monitor quality and efficiency results for assigned population.K.
Assists the Director of Quality in monitoring patient satisfaction
surveys, identifying problems and trends.L. Assists the Director of
Quality in enhancing systems for reporting adverse events, near
misses and potentially unsafe conditions.M. Works directly with the
staff toward improvement of identified processes.N. May assist with
chart audits relevant to advancing population health.0. Adheres to
standards regarding patient confidentialityAdministrative/Teaching
- 10%A. Assists in creating, trending and generating quality
reports.B. Attends any Quality meetings, staff meetings and
conferences as requested by the Director of Quality and CMO.C.
Works directly with the faculty/residents/staff toward improvement
of identified processes.Qualifications1.) Bachelor's degree of
Science in Nursing (BSN) AND2.) 1 year (12 months) of Quality
Improvement (QI) work experience in a clinical or healthcare
related field3) Registered Nurse with demonstrated clinical
experience.* All candidates must possess a Bachelor's Degree in
order to meet the Minimum Acceptable Qualifications.Condition of
Employment: Pursuant to the State Universities Civil Service
System, an out-of-state resident who is hired into this position
must establish Illinois residency within 180 calendar days of their
