Reg Full Time Professional Springfield, IL, USRequisition ID: 2619 Registered Nurse Illinois Crisis Prevention NetworkHope is a clinical, educational, and therapeutic provider for individuals with intellectual and developmental disabilities, including Autism Spectrum Disorder. The Illinois Crisis Prevention Network (ICPN) is the result of a collaborative relationship between Hope and Trinity Services, Inc. ICPN is designed to meet the behavioral and medical needs of people with intellectual and developmental disabilities who are experiencing significant behavioral and medical challenges. The ICPN consists of six statewide Support Service Teams who provide community-based support and consultation services to the person experiencing these challenges, as well as their family and or service providers.ICPN is currently seeking a Registered Nurse that will service all of our ICPN locations. This position can be based in any of the following: Rockford, Hillside, or Springfield, IL.WHAT YOU WILL DOReviews medical records and histories such as physician's orders, laboratory results, past hospitalizations and/or emergency placements, medications, patient responses, behavior, and other relevant documentation/records. May complete health care histories to determine those medical factors that may be influencing the individual's behavior. Based upon a review of medical history and related documentation, may make referrals for additional testing. May facilitate setting up medical appointments with a variety of medical practitioners.Provides health care related guidance and instructions to the individual and his/her family regarding care and treatment and, as appropriate, the use of equipment.As determined by the ICPN team, may make follow up visits to individuals to monitor their response to intervention plans that may include medical interventions.Participates in interdisciplinary team meetings, program staff meetings and Clinical Administrative Review Team Meetings (as requested) in support of the individual in crisis and their clinical/medical needs.Provides the individual and his/her family education and/or materials regarding identified health care issues/concerns.Communicates effectively with all ICPN team members, collaborative agencies, and community providers for the benefit and treatment of the client. Provides reports and other documentation in the prescribed time frames, and ensures accuracy and completeness of such reports.Maintains professional relationships, familiarity with current nursing policies and procedures, identifies and reports all potential safety hazards and attends mandatory in-services and trainings.Assumes responsibilities as a mandated reporter concerning incidents of purported abuse/neglect of clients as outlined in the agency's policies and procedures.Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.WHAT YOU WILL BRINGRequires graduation from an approved nursing education program resulting in an associate or diploma degree in nursing or a bachelor's degree in nursing.Requires licensure as a Registered Nurse in the State of Illinois.Successful background clearance, physical and drug/alcohol screening.Requires knowledge of professional nursing principles, concepts and practices.Requires knowledge of pharmacology of commonly prescribed medications and their therapeutic and possible adverse reactions.Requires ability to evaluate the physical condition and behavior for both youth and adult clientele and prepare accurate and detailed records of such an evaluation.Requires ability to instruct non-nursing personnel in the proper practices and procedures of healthcare.WHAT YOU WILL GETPaid holidays, sick days, vacation days, and personal days each year.Health, dental, vision, and life insurance.Short & long term disability.Tuition reimbursement & 401(k).On-site health clinic for covered employees.PI0bb569ea1b03-25660-34908642
